Our Payment Policy:
We accept the following methods for payments;
     
  • Cash
  • Checks, with proper photo Id
  • Visa, MasterCard & Discover Card
       

Appointment and Cancellation Policy:
We realize that your time is valuable, and in the spirit of efficiency we prefer that you call our office to schedule an appointment. However, if you have an urgent need to see Dr. Yarnevich during our office hours, we encourage you to come to the office.

Please try to give a 24 hour notification if you cannot make your scheduled appointment.

 

Medicare:
Dr. Yarnevich is an assigned Medicare Provider. The patient will be responsible for co-payments, deductibles and non-covered services. These charges are determined by Medicare. Once your deductible has been satisfied and if the charges are approved by Medicare, payments will be made directly to our office. If you have a secondary insurance, and Medicare is aware of this, in most cases they will crossover your claim to your secondary insurance. If they don’t forward this claim, then we will file the claim to your secondary insurance for you, unless otherwise specified by you the patient. Please be advised that there are services which may not be approved by Medicare. In those cases, you the patient will be responsible for the entire amount charged for the service not covered.

 

Medicaid:
Patients with Medicaid must present a current Ohio Medicaid Card before seeing the doctor. There are some services not covered by Medicaid for which the patient will be responsible.

       

Insurance Plans Where We Are A Participating Provider:
All co-pays are to be paid at the time of service.

     
 

HMO and Referrals:
Most HMO’s and Point of Service contracts need a referral from your primary care physician before you can see a specialist. Please call your insurance company 24 - 48 hours before your scheduled appointment to see if authorization has been placed (get authorization number). If authorization is not in place, you will need to reschedule.

 

3RD Party Insurance:
We do not bill 3rd party claims (such as auto insurance for motor vehicle accident). The patient is to pay at the time of service. A copy of the bill will be given to you for submission to the appropriate insurance company for reimbursement.

 

Minor Patients (under 18 years of age):
If a parent or legal guardian does not accompany a minor patient for an appointment, a dated note detailing requested services and giving Dr. Yarnevich permission to treat the minor, must be signed by the parent/legal guardian for placement in patient’s file.

 

Missed Appointments:
Unless cancelled at least 24 hours in advance, we have the option to charge for the missed appointment at the rate of a normal office visit. Please help us serve you better by keeping your scheduled appointment. If you show a pattern of missed appointments without proper notices you may be asked to seek your healthcare elsewhere.

Knowing Your Plan Requirements:
Patients should know the requirements of their individual insurance plans. This may mean that you need to contact your insurance carrier to check on their requirements and/or plan limitations.

You are required to know which hospitals or ambulatory surgery centers are covered by your plan prior to your initial visit with our physician so scheduling may be done at the correct facility. If precertification is required for a procedure we are performing, it is your responsibility to inform our office that precertification needs to be obtained for the procedure. Our staff will then contact your carrier to obtain the required precertification.

Your insurance policy is a contract between you and your carrier. We cannot guarantee payment of your claims and our office will not accept responsibility of negotiating claims with your insurance carrier or other facilities. You are ultimately responsible for payment of the services provided by our physician and facilities, regardless of the status of the insurance claim. If your carrier pays only a portion of your bill or rejects the claim, any contact for explanation should be made by you, the policy holder. Reduction or rejection of your claim by your carrier does not relieve you of financial obligation incurred with our office. If necessary we will lend a hand when needed to help process your rejected claim.
